Traumatic Pseudoaneurysm and Arteriovenous Fistula Among Interal and External Carotid Artery and Internal Jugular Vein -Case report-



Abstract
Recently, we experienced a case of traumatic pseudoaneurysm and arteriovenous fistula among internal and external carotid artery and internal jugular vein that was caused by gunshot injury. The patient was 22 years old man and suffered from
palpable
thrill and growing mass at right side neck that had happened after injury. The lesion involved distal internal carotid artery 5cm distal to the bifurcation of common carotid artery. We used vertical mandibulotomy for surgical exposure of the
lesion
and
did complete resection of the lesion and reconsttution of the circulation of internal and external carotid arteries with saphenous vein interposition graft. Postoperative course was uneventful. It's important to plan to approach to the highly
located
lesion of internal carotid artery preoperatively. We recommend the method of vertical mandibulotomy in case of highly located lesion of internal carotid artery.
